Instructions

• Open Enrollment is from January 1, 2012 through February 15, 2012
• Only one application is needed per family
• If you do not have an email address, please leave it blank
• If you have more than one student, click on "Add Student" for each additional student
• For each student select the grade the student will attend for the 2012-2013 school year
• If your student will not be 5 years old by September 1, 2012, select Pre-K for the grade
• Please include your student's full name, including middle name if applicable
• Please verify that you have entered the correct birthdate and grade for each student

Terms of Enrollment

• North Star Academy is a public charter school, and like all public schools is available to students of all ability levels, and will not discriminate in program benefits, participation, employment, or treatment on the basis of race, color, religion, national origin, or gender.
• North Star Academy will function under an open admission policy on the same basis as other public schools. If the number of students applying to enroll exceeds the capacity of the school, then those to be admitted shall be chosen at by lottery from among the applicants. Siblings of enrolled students are exempt from the lottery process.
• North Star Academy is in need of parents who are willing and able to serve the school community on a volunteer basis. I am willing to help where and when I am able.
• North Star Academy has a strict dress code and I will ensure that my students are in compliance with that policy.
• I understand that I will be responsible for providing transportation of my students to and from school each day.
• I understand that completing the application and agreeing to this terms will constitute my students' application for enrollment.
